In an effort to address the challenges and needs of our military personnel and their families, the Santa Clarita Adult Day Health Care Center is proud to announce the expansion of services to veterans.  If you are a veteran or caring for a veteran you will find the center’s day care therapeutic environment is designed to promote socialization and stimulation and maximize individual potential for independence, dignity and quality of life among functionally impaired adults.
The Santa Clarita Adult Day Health Care (the only one in the Santa Clarita Valley serving the Valley’s veterans and families) is well suited for those who have suffered a stroke, traumatic brain injuries and other physical, mental or memory impairments including Alzheimer’s and dementia.

Through both health and social models of care, the center helps delay or prevent premature placement in a nursing home, psychiatric facility or other institution for the frail elderly and other physically or mentally impaired adults 18 and over.

The Health Model incorporates the use of modified exercise programs, support and therapy groups, basic nursing care, supervision or observation, physical therapy, occupational therapy and speech therapy.  The Social Model uses modified exercise programs, recreational and creative activities, psychosocial support, and therapy groups and individual counseling as needed.

Our programs are staffed by a multi-disciplinary team of a registered nurse and social worker who evaluate each participant and customize their individual plan of care, providing the tailored care a veteran has earned.

The veteran must be registered with the Veterans Administration.

To register a veteran, contact the Veterans Administration Sepulveda Ambulatory Care Center (SACC) at 818-891-7711 ext. 9097 or the County of Los Angeles Department of Military and Veterans Affairs Santa Clarita Service Center at 661-254-3413. 

Once registered, obtain a referral for adult day health care from your primary care physician.
