• Convenient access to your primary care physician at either one of their offices located in Valencia, Lancaster and Palmdale, open 8 am-5 pm – Monday-Friday.  Primary care services include Family Practice, Geriatrics, Internal Medicine, Pediatrics and Women’s Health.  Heritage Sierra Medical Group also has several Independent Primary Care Physicians throughout the Santa Clarita Valley who practice out of their private offices; contact Member Services for more information.

• Patient Education, Telemedicine, Preventative Care, Disease Management and Dedicated Patient Advocates
• Local Santa Clarita Valley Specialty Care.  Heritage Sierra Medical Group offers an extensive range of local specialty care services accessible to you through their referral management services.
• In-Patient Care at Henry Mayo Hospital
• Urgent Care Centers throughout the Santa Clarita Valley including the walk-in option in their Primary Care office in Valencia, Monday through Friday, 8am-5pm.  Heritage Sierra Medical Group Urgent Care centers are capable of handling a variety of medical conditions such as:  colds, coughs, sprains, fractures, shortness of breath, etc.  If it is not a life threatening emergency, utilizing a contracted Urgent Care Facility rather than an Emergency Room will reduce your out-of-pocket costs, save you time and ensure you receive the immediate urgent care you need.
We are currently accepting new patients!  Heritage Sierra Medical Group is contracted with most major HMO, EPO, PPO, Covered California, Medicare and Medicare Advantage Health Plans.
